Swab count etc should be carefully adhered to during operation. Any doubt should be checked by x-ray.
Ligating ducts/ureters/arteries
Certain procedures do have high risk of such an error e.g. biliary surgery, colonic surgery, and laparoscopic sterilisation. The surgeon should be aware of the specific risks in each case and appropriate care taken.
